I heard this at the Easter Vigil service, where our Youth sang this beautifully! It's a combination of English and Latin. And the words are beautiful and so is the tune.
This tune transported me back in time, to our summer holidays during school, which were spent with my grand mom in Goa. We would recite the rosary at the local chapel during the month of May. And that is where we sung the Litany in Latin.
Thank you Matt Maher for bringing back those beautiful memories with this lovely tune.
Suggestions : In our church, our priests are quite particular about sticking to the words of the Roman Misal. So I'd suggest you drop the first verse and stick to "Lamb of God, you take away" as the introduction to all the three verses.
Here are the lyrics to it :

Lamb of God
Wonderful Counsellor,
Mighty God, and Prince of Peace,
Emmanuel, Morning Star,
King of Kings and Lord of Lords
Lamb of God, you take away,
The sins of all, the world
Miserere nobis
Bread of life, You take away,
The sins of all the world
Miserere nobis
Agnus Dei, You take away,
The sins of all the world
Dona nobis Pacem
Do give it a listen here :) Am sure you will love the tune.
